About the Resident & Family Portal and How to Enroll
Vantage Healthcare provides access to the Resident & Family Portal for residents or their family member or authorized representative for residents enrolled in the Chronic Care Management (CCM) program. Access to the portal provides for a more informative, engaging discussion with the CCM Nurse Care Coordinator about the plan of care.

Complete and submit the online enrollment form below

The authorized user will receive an email to activate. Follow the instructions in the email.
