About the Resident & Family Portal and How to Enroll

​Vantage Healthcare provides access to the Resident & Family Portal for residents or their family member or authorized representative for residents enrolled in the Chronic Care Management (CCM) program. Access to the portal provides for a more informative, engaging discussion with the CCM Nurse Care Coordinator about the plan of care.

Please note that at this time the Portal contains Physician and Nurse Practitioner/Physician Assistant notes only. A connection to Laboratory and Pharmacies for lab and medication lists are coming soon.

The authorized user will receive an email to activate. ​Follow the instructions in the email.
